Your first self-injection can feel intimidating. Most people discover it's far easier than expected - the needle is tiny, and after a few weeks, the process becomes second nature. This guide walks you through everything from preparation to disposal.
Before You Begin: What You'll Need
Gather everything before you start. You don't want to be searching for supplies mid-injection.
- Your tirzepatide pen - Remove from refrigerator 30 minutes early to reach room temperature
- Alcohol swabs - For cleaning the injection site
- Sharps container - Never throw needles in regular rubbish
- Clean, well-lit space - A bathroom counter or table works well
Pre-Injection Checklist

- ☐ Solution is clear and colourless (not cloudy)

Choosing Your Injection Site
Tirzepatide goes into the subcutaneous fat layer - the soft tissue just under your skin. Three areas work well:
Abdomen
Most common site. Use area at least 5cm away from belly button. Avoid the waistline where clothing may rub.
Front of Thigh
Middle third of the front thigh. Easy to see and access. Good alternative if abdomen is sore.
Back of Upper Arm
Fleshy area on the back. May need someone to help. Useful for rotation.
Rotation matters. Don't inject in the exact same spot each week. Use a different area within your chosen site, or alternate between sites (abdomen one week, thigh the next). This prevents lipohypertrophy - fatty lumps that can form from repeated injections in one location.
Step-by-Step Injection Process
Inspect the Medication
Look through the viewing window. The solution should be clear and colourless. If it's cloudy, contains particles, or looks discoloured, don't use it.
Attach the Needle (if separate)
Remove the pen cap. If using a pen with separate needles, attach a new needle by pushing and twisting clockwise until secure. Remove both needle caps.
Prime the Pen (First Use Only)
For a new pen, set the dose to 2 units, point upward, and press the injection button. A drop should appear at the needle tip. This removes air bubbles.
Set Your Dose
Turn the dose selector to your prescribed dose. You'll hear a click for each unit. The dose display will show your selected amount.
Clean the Injection Site
Wipe your chosen area with an alcohol swab in a circular motion. Let it air dry for 10 seconds - injecting into wet skin can sting.
Insert the Needle
Pinch a fold of skin with one hand. With the other, insert the needle at a 90-degree angle (straight in) in one smooth motion. You can let go of the skin fold.
Inject the Medication
Press and hold the injection button until the dose counter returns to "0". Keep holding for 10 seconds after - this ensures full delivery.
Remove and Dispose
Pull the needle straight out. Press a cotton ball or tissue against the site if there's any bleeding. Replace the outer needle cap and dispose in your sharps container.
Timing Your Injection
Take tirzepatide once weekly, on the same day each week. The specific time of day doesn't affect how well it works - choose whatever time you'll remember consistently.
- With or without food - Doesn't matter for absorption
- Morning or evening - Some prefer evenings so any initial nausea happens during sleep
- Set a reminder - Phone alarm on your chosen day helps maintain consistency
What If You Miss a Dose?
Missed Dose Guidelines
- → Within 4 days: Take your dose as soon as you remember. Continue with your regular schedule.
- → More than 4 days late: Skip the missed dose. Take your next dose on the regular scheduled day.
- → Never double up: Don't take two doses to make up for a missed one.
If you want to change your injection day, you can do so as long as your last dose was at least 3 days ago. For example, if you've been injecting on Mondays but want to switch to Fridays, simply take your next dose on Friday instead.
Common Injection Concerns
"It hurts when I inject."
Try different sites - some areas have more nerve endings than others. Ensure the pen has warmed to room temperature. Insert quickly rather than slowly. If pain persists, discuss with your healthcare provider.
"I see a drop of medication on my skin after."
Small amounts can leak out - this doesn't mean you need a larger dose. Make sure you're holding the button for the full 10 seconds after the counter reaches zero.
"There's bruising at the injection site."
Minor bruising is normal and harmless. Avoid injecting in bruised areas. If bruising is excessive, you may be hitting small blood vessels - try a different spot.
"I'm afraid of needles."
The needle is very thin - much smaller than those used for blood draws. Many people find that looking away during insertion helps. The actual injection takes only seconds.
Safe Disposal in Nigeria
Used needles require proper disposal. Never throw them in regular waste where they could injure sanitation workers or be reused.
- Use a dedicated sharps container or a thick plastic bottle with a secure cap
- When container is three-quarters full, seal it completely
- Return to your pharmacy or healthcare provider for proper medical waste disposal
